One-Time Movement Workshop
Workshop: Monday Moves
Date: March 2, April 6, and/or May 4, 2026
Facilitator: Heather Winia
I voluntarily choose to participate in a movement-based workshop facilitated by Heather Winia.
I understand the Workshop involves physical movement, including dancing and rhythmic activity, and that participation carries inherent risks, including possible physical injury or emotional discomfort.
I affirm that I am physically and mentally able to participate, will choose my own level of involvement, and may stop or modify my participation at any time.
I knowingly and voluntarily assume all risks related to my participation, whether known or unknown, to the fullest extent permitted by Michigan law.
In consideration of being allowed to participate, I hereby release and waive any and all claims against Heather Winia and the Saugatuck Center for the Arts, including claims based on ordinary negligence, arising from my participation in the Workshop, to the fullest extent permitted by Michigan law. This release does not apply to gross negligence or willful misconduct.
I understand this Workshop is not medical or mental health treatment.
This Agreement is governed by the laws of the State of Michigan.
I have read this Agreement, understand it, and understand that selecting "Yes, I agree," below will serve as my digital signature on this agreement.
